Parish Registration Form

New Parishioner Registration Form  - Join the Saint Mary Parish Family; one form per family.

CCD Registration Forms

New Religious Education Student Form - One copy of this form must be completed for each child the first year you register you child for Religious Education at St. Mary's Parish.  This form is not necessary to register a child in successive years. 

Renewal Religious Education Registration Form - Form we need every year where we register the entire family with minimal work

Please Note:

No registrations are processed with out payment or prior arrangements with Mary Williams the DRE.  To mail them in send to:  St Mary Religious Education, PO Box 396, Georgetown, MA 01833.  Please do not send cash or leave registration forms with cash in the mail box.
